Thank you for considering our services.  My name is Dr. Joshua Shifrin, and I offer comprehensive neuropsychological evaluations.  I do want to let you know, however, that a neuropsychological evaluation is very thorough.  There is some paperwork, an intake phone call, about five hours of testing either in person in our Livingston, NJ office and/or by Zoom, some rating scales to fill out, a lengthy report, and a feedback session.  The cost is $4,500, and I am out of network.  Some of the costs may be reimbursable through your out-of-network benefits.  If you’re interested in exploring this option, please call the number on your insurance card and let the representative know that the patient requires a neuropsychological evaluation, the doctor is out of network, the cost is $4,500, and you would like to know how much of this is reimbursable through your out-of-network benefits. Please give the representative the following four codes:

With this information, the representative should be able to tell you over the phone how much of this is going to be reimbursable.  Half of the payment ($2,250) is due at the time of testing.  The balance is due when the report is completed.  We do, however, offer payment by credit card and/or payment plans.  Once the report is completed, and the payment is made in full, my office manager will provide you with the receipt needed for reimbursement purposes.  Please feel free to email drshifrin@yahoo.com or call me at (860)966-0309 with any other questions.  We offer testing during the week or on weekends and are accepting new patients.
